The expert dermatologist examined the clinical differentiation between topical and systemic JAK inhibitors, noting that oral agents are particularly suitable for patients with large body surface area (BSA), numerous small lesions, or rapidly progressive disease. While all JAK inhibitors carry class warnings for serious adverse events (AEs), Dr. Rosmarin highlighted that the vitiligo population may have a lower underlying risk for these events compared to other autoimmune populations. He emphasized that topical JAK inhibitors have demonstrated a favorable safety profile in clinical practice, while the upcoming oral options will complete the clinician's armamentarium for managing more extensive disease.
